Healthy Opportunities Survey
The Department of Social Shrives is conducting a Healthy Opportunities Assessment survey. The brief survey will provide Department of Social Services with critical data to evaluate communities across the state.
This survey does not impact benefits and is not collecting personal information. This information is only being used to analyze community trends and needs.
The survey will be active until May 31, 2020. On June 1, 2020, the survey will close and be analyzed. The results of the survey will be shared with interested parties. Click the link below to take the survey.
